A Secret Federal Unit is Taking Down Billion-Dollar Medicare Fraud Syndicates
Fraudsters stole $100 billion from Medicare and Medicaid last year – and most of them never saw a single federal agent at the door.
Now a little-known unit inside the federal government is changing that – and the crime syndicates running the biggest schemes are finally getting knocked.
What agents found when they finally knocked on the right door leads straight back to Democrat donors.
Inside The Knock: The HHS Unit Clawing Back Billions in Medicare Fraud
Tucked behind blacked-out windows in a suburban Maryland medical plaza, the National Training Operations Center – agents call it “The Knock” – looks nothing like a federal law enforcement facility.
Every surface is covered with ordinary objects – lamps, filing cabinets, reception desks – all of it foam, all of it engineered to train agents for the most sophisticated healthcare fraud takedown operation the federal government has ever assembled.
The New York Post got an exclusive tour this week of The Knock, the high-tech headquarters of the Department of Health and Human Services Office of Inspector General – HHS-OIG – the federal agency that operates almost entirely outside public view while clawing back billions stolen from Medicare and Medicaid every single year.
Wall-mounted screens display live surveillance feeds from a dozen locations simultaneously – one fixed on a California cul-de-sac, another on an Ohio apartment complex, a third watching a Manhattan storefront.
“A lot of what we’re doing is proving a negative,” one federal agent told the Post as he embedded a camera the size of a pinhole into a household object. He pointed to the California feed – a doctor’s car in the driveway during clinic hours. “He’s billing for seeing patients right now. He’s actually at home.”
That is fraud. And HHS-OIG has 1,450 employees – including more than 100 data analysts trawling through trillions of dollars in billing records – looking for exactly that.
$45,000 a Day Per Agent – While Democrats Called Dr. Oz’s Medicare Crackdown Political
Assistant Inspector General for Investigations Scott Lampert ran the numbers.
Each of his investigators recovers an average of $45,000 per day in stolen taxpayer funds – $15 million a year per agent. The agency’s most recent semiannual report confirmed HHS-OIG returned $12.70 for every dollar spent on enforcement between October 2025 and March 2026, recovering $5.56 billion in that single six-month window.
“If I had 100 more agents, think about what I could do,” Lampert told the Post. “If HHS-OIG was on the stock market, I would invest in us.”
Medicare and Medicaid together hemorrhage an estimated $100 billion a year to fraud – and HHS-OIG is chronically understaffed to stop it.
Dr. Mehmet Oz, Trump’s CMS administrator, stood at the White House in February alongside Vice President Vance and HHS Secretary RFK Jr. to announce the administration’s CRUSH initiative – Comprehensive Regulations to Uncover Suspicious Healthcare.
The announcement included a nationwide moratorium on new Medicare enrollment for medical equipment suppliers and the deferral of $259.5 million in Medicaid funding to Minnesota pending fraud investigation.
“CMS is done trying to catch fraudsters with their hands in the cookie jar – instead, we’re padlocking the jar and letting them starve,” Oz said.
Democrats called it political. The fraud evidence called it overdue.
Democrat Donors Busted in Medicaid Kickback Scheme Targeting Seniors
The Brooklyn case that dropped this week is where the Democrat donor trail starts.
Pervez Siddiqui – 78 years old, owner of 15 New Jersey pharmacies, board chair of a Pakistani-American nonprofit, and a prolific donor to Democrat campaigns – was arrested alongside seven co-conspirators on charges of running a $38 million Medicaid kickback scheme through two Brooklyn social adult day care centers.
The operation was brazen. Paid recruiters worked senior housing complexes, transit stops, and medical waiting rooms – anywhere they could find elderly residents on Medicaid. Those seniors were signed up to “enroll” in APNA Adult Daycare or Ashiana Social Adult Daycare, received $500-a-month cash kickbacks, and rarely or never set foot inside the facilities.
Some enrolled patients weren’t even in the country. “The patient is in Pakistan, they’re in Morocco. They’re not even in this country,” a source told the Post.
Proceeds moved through shell companies – payments coded under words like “medicine” or “laddu,” a South Asian term for sweets, mixed in among fake “gifts” and “dividends” to disguise the flow.
Siddiqui had spent years embedded in the city’s Democrat power structure – fundraisers, checks to campaigns, a sit-down with incoming Mayor Zohran Mamdani just weeks before the indictment dropped.
Prosecutors already knew the playbook.
Brooklyn alone produced a $68 million Medicaid fraud conviction last year and a $10.6 billion urinary catheter racket run by the Russian mob out of a Gravesend storefront – one supply company billed $250 million for catheters in 2023 alone, $50 million more than every other medical provider in the country combined.
Queens added a $120 million Medicare and Medicaid scheme in February 2026, two men running fake day care centers that claimed over a thousand patients on days their certificate of occupancy allowed 81.
The June 2026 National Health Care Fraud Takedown charged 455 defendants in 56 federal districts across 45 states and territories – including 90 doctors – tied to $6.5 billion in false claims.
The year before, the 2025 Takedown broke every record in DOJ history: 324 defendants, $14.6 billion in intended losses – double the previous record.
For decades the money kept flowing out and the enforcement budgets stayed flat – while Democrat politicians called every crackdown political theater.
Trump’s team is delivering results. The criminals who spent years stealing from seniors while donating to the politicians who protected them are finally learning what The Knock actually means.
